COCONINO COUNTY — In April, Coconino County Health and Human Services (CCHHS) launched the Community Health Needs Assessment (CHNA) survey to help identify the health priorities, community needs, strengths and risk factors within Coconino County. To ensure that everyone has an opportunity to share their input, we’ve extended the deadline to take the survey to Saturday, Oct. 31.
The CHNA is a routine, evidence-based process used by public health and community action agencies to identify the most pressing health issues and unmet needs impacting local communities. Input gathered from the survey will guide planning efforts, support funding opportunities and inform decisions about resources, programs and long-term public health strategies.
CCHHS encourages residents of all ages, backgrounds and neighborhoods to take part. By sharing their experiences, individuals help build a healthier and more responsive community for everyone.
