COCONINO COUNTY — Coconino County Health and Human Services is seeking a Health Start Case Worker.
The Health Start Program is an evidence-informed home visiting program that provides home-based education, support, and advocacy services to pregnant/postpartum people and their families in the northern region of Coconino County.
The Health Start Program team consists of a Program Coordinator, two (2) full-time Case Workers. Case Workers’ carry a caseload of 25 families and conduct weekly in-home visits.
